This is the consent form that I will give to Jodie Coote, my actress, to make sure that she understands what will happen with the horror opening that she in involved with and whether she agrees to what she has to do and where the horror opening will go.

This is the consent form that I will give to Nick Brewer, my actor, to make sure that he understands what will happen with the horror opening that he in involved with and whether he agrees to what he has to do and where the horror opening will go.

This is the costume and make-up document that I will give to Jodie. I did this so she understands clearly what her costume is like and I added a picture of the make-up so that she understands that her make-up will not be a normal everyday make-up she is used to having on her face.

This is the costume and make-up document that I will give to Nick. I did this so he understands clearly what his costume is like and I added a picture of the inspired make-up look so that he understands that he will have make-up on his face and so he gets a rough idea of what the make-up will look like.

This is the document that I will give to Jodie, so that she understands what each shot will involve and what she has to do for each individual shot on both of the days that I am filming. I have added a diagram to one of the shots so that she gets a better understanding of what location I am talking about.

This is the shooting schedule that I will give to both Nick and Jodie so that they understand what they need to bring, when they need to be at the certain locations and where we will meet. I added Jodie's day into Nick's shooting schedule so that he knows that there is going to be another filming day that he doesn't appear in the shots, but he may come if he wants to see what Jodie's shots are.
